ILLINOIS NAME CHANGE FORMS: THE ADULT NAME CHANGE PACKAGE:

The principal forms required for an Illinois Adult Name Change are all available for download on our site. The major Illinois Name Change forms include the following:

  1. Notice of Filing Petition for Name Change
    Information Required on This Form: date on which you will be filing the form, which must be six weeks after the date that you publish the notice in a newspaper; your present full name; the new name that you want to use; courthouse location; address of courthouse;

Fourth Circuit - The counties of Clinton, Marion, Clay, Fayette, Effingham, Jasper, Montgomery, Shelby and Christian.
Fifth Circuit - The counties of Vermilion, Edgar, Clark, Cumberland and Coles.
